Once you click on SUBMIT, you will have access to our secure client portal to submit your new client forms.

IMPORTANT: If you do not submit your client forms 24 hours PRIOR to your scheduled appointment,

your appointment will be rescheduled or canceled. New clients will not be seen without completed client forms on file.
